Informed consent 

 

I understand that the purpose of the exercise session is to provide safe and effective exercise to improve health and fitness. Exercises may include: 

  • Warm-up activities involve gentle cardiovascular exercises such as walking, jogging, jumping, lunging and turning (this could mean a variety of age-appropriate accessories).

  • Muscular strength and endurance activities using body weight. Motor fitness skills such as balance, coordination and flexibility (this could involve a variety of age-appropriate accessories).

  • Cool-down and developmental flexibility exercises to improve movement around the joints and range of motion

 

The instructions and advice presented are in no way intended to be a substitute for medical counselling/advice. I understand that exercises are designed for the attendants' needs to avoid injuries. 

 

By booking and confirming the attendance to the session, I assume there is a low risk of injury resulting from performing the exercises shown, and the instructor would be only liable if there is a breach of duty of care. 

I understand that a programme of regular exercise is beneficial. Some of these benefits include a decrease in risk of heart disease, a decrease in body fat; improved blood pressure; improved psychological function (including self-esteem); and enhanced aerobic fitness.

 

 

Physical Activity Readiness Questionnaire (PAR-Q) 

I declare that I, or the person under my guard, am in good health and would not perform the activity if having any of the following health situations:

 

  • Heart conditions which could impede physical activity

  • Chess pain when performing physical activity

  • Chess pain in the past month, even when not performing physical activity

  • Dizziness that could cause losing balance or consciousness

  • Bone or joint problems that could get worse with changes in the performance of a physical activity

  • The participant is not taking medication for blood pressure or heart conditions

  • There is no other reason why the participant should not do physical activity

 

My confirmation of participation in the Pilates sessions acknowledges my consent to be videotaped during the class for internal use. I waive any right to inspect or approve any images or recordings and ownership rights. All this, in the understanding that the organisers would not use my photo for any publicity without my final authorisation, and they are following the General Data Protection Regulation (UK- GDPR) enforced in the United Kingdom, and they will protect my data according to the regulations in place.
